The Valley Springs Monthly Immunization Clinic

Valley Springs, CA...Due to low attendance at the Public Health Services’ Valley Springs, Immunization Clinic, Public Health Services will no longer offer immunizations at the United Methodist Church at 135 Laurel, Valley Springs, effective July 1, 2014. The immunization clinics in San Andreas will continue as follows...






NEW CLINIC AND FEE SCHEDULE

3:00 pm – 5:30 pm

8:00 am – Noon

The fee for each immunization is $26.00

No one who is eligible to receive vaccine will be denied because of inability to pay

Appointments are not needed

Eligibility for Public Health Services immunization clinics:

Eligible – Public Health provides immunizations to the following:

Children and youth under 19 years of age without private health insurance

Children and youth under 19 years of age whose insurance does not cover vaccinations

Children and youth under 19 years of age with Medi-Cal

Pertussis and Flu vaccine are available to all persons without restrictions

Not Eligible – Public Health cannot provide immunizations to the following:

Children and adults with private insurance that covers vaccinations

Seniors with Medicare Part D

Adults 19 years and older with Medi-Cal coverage
